Our CCM department is headed by Dr. Chetana Kriplau, MD, a board-certified family physician serving as the Clinical Oversight for Care Management at Trinity Medical Associates. She acts as a liaison between providers and care coordinators to ensure seamless, high-quality patient care. Dr. Kripalu provides guidance and support in addressing clinical questions and care management issues, developing plans of care, helping to enhance outcomes, and improve the overall quality of care

Trinity offers extended services for their patients with multiple chronic conditions. CCM (chronic care management) is a program that was created by the Centers for Medicare and Medicaid Services and by using strict medical guidelines. This program is endorsed by the American Medical Association and each of the major medical societies. CCM has been identified by the U.S. Department of Health and Human Services as “a critical component of primary care that contributes to better health and care for patients, and even caregivers.”
